How to Fill Out Form WH-347, Column by Column
Updated November 6, 2026. General information, not legal advice.
Form WH-347 is the Labor Department's optional form for weekly certified payrolls on Davis-Bacon work. The form itself is optional, but the weekly payroll submission isn't. This guide follows the official instructions, which are the final word.
Top of the form
Mark whether you're the prime contractor or a subcontractor, and tick the final payroll box in the last week of work. Then enter the project name and number, location, wage determination number, week ending date and your business name and address. The certified payroll number starts at 1 and goes up each week.
The columns
| Column | What goes in it |
|---|---|
| 1A Entry no. | Number workers in order, starting at 1. Keep counting on extra pages, so row 1 of the next sheet may be entry 9. A worker with two classifications keeps one entry number. |
| 1B to 1D Name | Last name, first name, middle initial. |
| 1E Identifying no. | The last four digits of the Social Security number, or another number specific to the worker. Never the full Social Security number. |
| 2 J or RA | J for journeyworker. RA for a registered apprentice, plus their level of progression. |
| 3 Classification | The classification for the work actually performed, as listed in the wage determination. Use a separate row for each classification worked. |
| 4 Hours each day | Day letters and dates go across the top. Enter project hours as straight time or overtime. The workweek doesn't have to start on Sunday. |
| 5 Total hours | The total of the hours in column 4. |
| 6A Hourly rate | The actual straight-time rate on top and overtime rate below. Don't include cash paid in lieu of fringe. |
| 6B Fringe credit | Total hours times the total hourly credit from the fringe table on page 2. |
| 6C Payment in lieu | Total hours times the hourly cash paid in place of fringe benefits. |
| 7A Gross, this project | Gross earned this workweek for hours on this project. |
| 7B Gross, all work | Total gross for all work this week, if they also worked elsewhere. |
| 8 Deductions | Tax withholdings, FICA, other, and total. Show actual deductions from gross for all work. Don't pro-rate them across projects. |
| 9 Net pay | The actual amount paid for all work that week. |

Boxes 1, 2, 3 and 6 must always be checked. Box 4 applies if anyone was paid as an apprentice, and each program must be listed. If it doesn't apply, leave it unchecked and enter N/A. Box 5 covers fringe benefits. If you claim a fringe credit, complete the hourly credit table. If you pay only cash, check box 5 and leave that table blank.
"Other" deductions must be explained in Additional Remarks. If "Other" combines more than one deduction, attach an addendum listing each one.
The certifying official then signs. Legally valid electronic signatures are acceptable, but scanned or photocopied signatures aren't.
